Особливості застосування мелатоніну пролонгованого вивільнення у хворих на цукровий діабет 2-го типу

Резюме

Цукровий діабет (ЦД) та пов’язані з ним ускладнення є одними із головних причин смертності в усьому світі. За даними Міжнародної діабетичної федерації очікується, що до 2045 року від цукрового діабету страждатимуть близько 700 мільйонів осіб. ЦД є прогресуючим хронічним захворюванням, яке вражає різні системи організму і може призвести до серйозних ускладнень фізичного здоров’я. Показано, що ЦД 2-го типу (ЦД 2) пов’язаний з більшою частотою розладів сну, які можуть бути наслідком самої хвороби або виникати через вторинні ускладнення чи супутні захворювання, пов’язані з діабетом. Разом з тим епідеміологічні дані свідчать про вищий ризик ожиріння, метаболічного синдрому та ЦД 2 в осіб із меншою тривалістю та поганою якістю сну. Тому у межах комплексного медичного обстеження при наявному ЦД 2 і для груп ризику його розвитку рекомендованими є оцінка якості сну, його розладів та їх фармакологічна корекція. Серед фармакотерапевтичних засобів для лікування порушень сну особливу увагу привертає мелатонін (N-ацетил-5-метокситриптамін) — індоламін, гормон епіфіза, який синтезується в організмі людини та назва якого пов’язана зі здатністю гормона агрегувати гранули пігменту меланіну. Встановлено, що мелатонін безпосередньо бере участь у багатьох біологічних процесах та обмежує окиснювальний стрес як позаклітинно, так і внутрішньоклітинно. Ендогенний мелатонін має лінійну кінетику, період напіввиведення звичайної форми мелатоніну зі швидким вивільненням перебуває в межах від 45 до 65 хв, він швидко метаболізується і повністю виводиться через 3–4 години. Наразі представлені форми мелатоніну пролонгованої дії у дозуванні 1 та 2 мг, які забезпечують повільніше та триваліше всмоктування, пікова доза в них відстрочена та має меншу величину, а рівні підтримуються від 8 до 10 годин. Це відповідає кривій фізіологічної секреції ендогенного мелатоніну. Оскільки доза мелатоніну у формах із подовженим вивільненням значно менша, ніж у формах зі звичайним вивільненням, це допоможе знизити ймовірність появи побічних ефектів, у тому числі у хворих на ЦД 2 та у групах ризику його розвитку.

Diabetes mellitus (DM) and its complications are one of the leading causes of death worldwide, with approximately 700 million people expected to suffer from DM by 2045, according to the International Diabetes Federation. DM is a progressive, chronic disease that affects various body systems and can lead to serious physical health complications. Type 2 diabetes mellitus (T2DM) has been shown to be associated with a higher incidence of sleep disturbances, which may be due to the disease itself or secondary complications or comorbidities associated with DM. However, epidemiological evidence suggests a higher risk of obesity, metabolic syndrome, and T2DM in individuals with shorter sleep duration and poor quality of sleep. Therefore, an assessment of the quality of sleep, its disorders and their pharmacological correction is recommended as a comprehensive medical examination for existing T2DM and for groups at risk of its development. Among pharmacotherapeutic agents for the treatment of sleep disorders, melatonin (N-acetyl-5-methoxytryptamine) attracts special attention. It is indoleamine, a pineal hormone synthesized in the human body, which name is related to the ability to aggregate melanin pigment granules. It has been found that melatonin directly participates in many biological processes and limits oxidative stress both extracellularly and intracellularly. Endogenous melatonin has linear kinetics, the half-life of the usual form of melatonin with rapid release ranges from 45 to 65 minutes, it is quickly metabolized and is completely excreted after 3–4 hours. Currently, 1 and 2 mg prolonged-release forms of melatonin are available, which provide slower and longer absorption, a delayed and lower peak dose, and levels maintained for 8 to 10 hours — similar to the physiological secretion curve of endogenous melatonin. Since the dose of melatonin in prolonged-release forms is much lower than in immediate release forms, this will help reduce the risk of possible side effects, including in patients with T2DM and in groups at risk of its development.
